It has been an honor to be in private practice for 30 years, and the last 18 years with my friend Dr. Francis. Our goal was to retire here, treating generations of families with the care for which we have become known. However, due to changes in our ability to practice medicine in our own way, changes in local market insurance, and daunting, mandatory government requirements, we are unable to continue in the business of private practice. It is with heavy hearts that we write to let you know of our decision to close Family Medicine Specialists.
Our last day will be April 3, 2019.
We cannot begin to tell you what a blessing our patients and staff have been and just how much we have enjoyed caring for you and your families over the years. God gave us a season, and now it is time for a season of change. To go where God has planned for each of our Providers.
Dr. Francis will be retiring from private practice and plans to keep up with medicine working with our Vets at the VA. Janene will be working with Dr. Francis at the VA, Tera will be starting a new job in New Mexico, and I will continue on working with nursing home communities.
We will transfer records to a practice of your choice up to April 5, 2019. At that time all medical records will be turned over to a custodian for Medical Records. We will post the location at a later date.
If you chose not to transfer to the custodial, we understand and will help you with your record transfer to a location of your choice. If you do not request records prior to April 5th, you will need to contact the custodian. Due to the extensive anticipation of record copying there will be a fee applied.
We will transfer your records within the state mandated 30 days from your request. A signed authorization will be required. We have them at the front desk or you can obtain one at your new office. By law, all medical records are confidential, and will not be released except by a signed authorization by the patient or guardian.
We understand that many of our patients have medical needs that need to be followed on a regular basis and medications that will need refilled after we are closed. We ask that you take action to find a new physician and establish with an office visit to avoid any gaps in care and medication management. We will not be able to fill medications after April 3, 2019.
Again, it has been a great joy being a part of your lives.
We wish you the best of health and will miss this part of our life.
